Grant applicants must express the intention of applying for a Caring Community Grant Award by Jan. 8, 2011.

Format requirement for letters of intent for the 2011-2012 grant cycle is as follows (applicants will provide information responding to these points). Letters of intent should be conveyed to District CEO Lee Michelson at 525 Veterans Blvd., Redwood City, 94063, or via email to lmichelson@sequoiahealthcaredistrict.com
