The Sequoia Healthcare District’s 2015-16 support is for two new efforts. The District’s funding is being used for:
- expand the Ron Robinson Senior Care clinic to the South County Campus. The special clinic offers a unique comprehensive approach to caring for older adults with significant health problems.
- support a hospital to home transition program. Patients leaving the Medical Center’s hospital in San Mateo will receive 30 days of supportive services such as Meals on Wheels in an effort to better help them recover and hopefully eliminate the need for re-admission.
