Support a Public Health Nurse at Belmont Apartments to provide medical case management, medical consultation and medical liaison services and to improve access to and use of primary care medical services.

What it is: Formerly homeless, mentally disabled residents of Belmont Apartments are prone to misdiagnosed medical problems because of lack of access to primary health care and communication difficulties. They can also accumulate medical prescriptions, which can lead to incorrect dosages. Public Health Nurse supervision at the facility has reduced these instances and has helped clients obtain suitable and adequate care. The grant helps continue this program.
