What it is:
Transitions at Mission Hospice collaborates with churches, faith-based organizations, senior service centers and other venues to support and assist seniors with life-limiting illnesses who live at home. Tapping trained volunteers, the program fills service gaps by providing emotional support, transportation, important errands, household management, companionship and caregiver respite.
